Diocesan Safe Environment
The Diocese of Owensboro requires parent volunteers, school employees, coaches, etc., to participate in a Safe Environment session once every five years.
We encourage all parents to participate in a session online so they are able to volunteer in the classroom, chaperone field trips, etc. Please click on the Safe Environment Link.

OCS and OMHS Medication Guidelines
If you are sending medication to school with your child, the Medication Consent & Release (2 pages) must be filled out for each medication being sent.
You can utilize a feature using Adobe to type the information on the forms prior to printing them.

Please bring the following items with you for registration:
- All Parish Sponsored Families
- Please bring your Parish Voucher. This is very important.
- If you do not have this, you will be charged the Non parish sponsored tuition rate
- New Families (Grade 1-12)
- Baptismal certificate (if Catholic)
- OCS will request student records from previous school
- New Familieis (Kindergarten)
- Baptismal Certificate (if Catholic)
- Birth Certificate - original (we will make a copy for school file)
- Social Security Card - original (we will make a copy for school file)
- Kentucky Medical Examination Form - dated within 12 months prior to admission to school
- Kentucky Immunization Form
- Kentucky Eye Examination Form - dated within 12 months prior to admission to school
- Families who have not Pre-Registered
- Completed Pre-Registration Form with $50 pre-registration fee
