Diocesan Safe Environment
We encourage all parents to participate in a session online so they are able to volunteer in the classroom, chaperone field trips, etc. Please click on the Safe Environment Link.
- All employees of the Diocese of Owensboro must take Safe Environment Training and have a Background check every 5 years. Other diocesan policies for hiring of employees must also be followed.
- All adult volunteers in the Diocese of Owensboro who work with children and youth must take Safe Environment Training and have a Background check every 5 years.
- All children registerd in Catholic Schools or Parish Faith Formation programs must receive Safe Environment training each year, unless their parent/guardian have signed a formal opt-out form.

OCS and OMHS Medication Guidelines If you are sending medication to school with your child, the Medication From Home Consent & Release must be completed.

Please bring the following items with you for registration in July 26, 2011:
- All Families
- must have a payment plan in place
- New Families (Grade 1-12)
- Baptismal certificate (if Catholic)
- OCS will request student records from previous school - complete the Information Transfer sheet and OCS will make the transfer request.
- New Familieis (Kindergarten)
- Baptismal Certificate (if Catholic)
- Birth Certificate - original (we will make a copy for school file)
- Social Security Card - original (we will make a copy for school file)
- Kentucky Medical Examination Form - dated within 12 months prior to admission to school
- Kentucky Immunization Form
- Kentucky Eye Examination Form - dated within 12 months prior to admission to school
- Kentucky Dental Screening Form
- Families who have not Pre-Registered
- Completed Pre-Registration Form with $50 pre-registration fee
